Your card will not be charged at this time. By submitting this form, you authorize Asia Pacific Aesthetics to keep your card securely on file. Appointments cancelled with at least 24 hours notice are never charged. A missed appointment or late cancellation may incur a fee of up to $500, charged to the card on file. Your card may also be applied toward your treatment on the day of your visit.
